Preliminary and periodic medical examinations are one of the most important measures aimed at preserving the life and health of employees in the course of their work. Their goal is to establish the suitability of a person for the work performed, or the conditions in which they will have to work, to carry out the prevention of occupational diseases in a timely manner and to prevent accidents.

Attention! Responsibility for organizing and financing mandatory medical examinations lies with the employer, who, in case of failure to comply with the established procedure, may be held administratively liable.

In accordance with Article 213 of the Labor Code of the Russian Federation, workers engaged in hard work and work with harmful and (or) dangerous working conditions(including underground works, as well as work related to traffic), undergo mandatory preliminary (when applying for a job) and periodic medical examinations.

Another category of persons undergoing a mandatory preliminary examination are workers in the food industry, public catering trade, water supply facilities, medical organizations and children's institutions.

A medical examination is an event regulated by law, associated with the execution of mandatory documentation.

One of the documents that should be developed in preparation for medical examinations is list of contingent of workers subject to preliminary and periodic medical examination. This is a local regulatory act of the organization, which contains information about the professions (positions) of employees who are required to undergo medical examinations.

Its form is not approved by law, clause 20 of the Order dated 12.04.2011 N 302n “On approval of lists of harmful and (or) hazardous production factors and work, during the performance of which mandatory preliminary and periodic medical examinations (examinations) are carried out ...” (hereinafter Order No. 302n) stipulates only what mandatory data is indicated in it.

And so, in the contingent of workers subject to preliminary and periodic medical examinations, the following are indicated:

1. the name of the profession (position) of the employee according to the staffing table.

The list of contingents is developed and approved once, before any changes in the organization (reorganization, commissioning of new jobs, improvement of working conditions). Name lists based on the contingent will be developed annually.

Since 01/01/2012, the procedure for coordinating contingents and name lists with the bodies of Rospotrebnadzor is not provided, while the employer is obliged, within 10 days from the date of approval of the list of contingents, to send it to the territorial body of the federal executive body authorized to implement the federal state sanitary- epidemiological surveillance at the actual location of the employer. Thus, you can personally transfer the list of contingents to Rospotrebnadzor, while on your copy the representative of Rospotrebnadzor must mark that he received one copy, or send it by registered mail. Appendix 1 lists harmful production factors by profession. Most of it is occupied by chemical factors. There is an important clause in order No. 302n. Only for five physical factors specified in Appendix 1, MO is carried out: these are noise, sensory, physical overload, infrasound and light environment. But for these factors, a medical examination of personnel is carried out only if a class of dangerous or harmful working conditions is established at the workplace. If the working conditions are optimal or acceptable, a medical examination for this "five" is not carried out. For all other factors specified in Appendix 1, MO is carried out only on the basis of their availability at the workplace.

Let's look at an example

The work of the driver is connected with driving a vehicle, as well as with professional contact with gasoline vapors, with gasoline and industrial oils, with exhaust, that is, exhaust gases that have passed through the entire power supply system of the car.

Therefore, according to Appendix 2, paragraph 27 is suitable for the driver, and according to Appendix 1, you need to look at what exhaust gases and gasoline vapors consist of.

Gasoline is a hydrocarbon, gasoline vapors are hydrocarbon components (that is, mixtures), and this is paragraph 1.3.5 and aldehydes (aromatic substances). Gasoline is burned, therefore, we apply paragraph 1.2.37 - carbon monoxide. By looking at Annex 1, it can be determined that aliphatic aldehydes are included in the exhaust gases (paragraph 1.2.2).

When the SOUT passes, then the class will be set, but, as a rule, in modern cars the noise level is in the remote control, this is not a workshop, so the driver does not need headphones, and we do not write this factor.

In addition, the driver is constantly under the influence of vibration - general (paragraph 3.4.1) and local (paragraph 3.4.2). General vibration affects the body as a whole and is especially felt when working on a truck.

If goods or people are transported in a van or in a body, the oscillatory circuit is associated with road roughness (shaking) and kinetic movement. The human body, due to the presence of soft tissues, bones, joints, internal organs, is a complex oscillatory system, the mechanical response of which depends on the parameters of the vibrational impact.

Where to send the list of contingent

The document is sent to Rospotrebnadzor in a notification manner, no approval is required. Send it by letter with notification within 10 days after approval by the director of the company to the territorial body of Rospotrebnadzor at the actual location of the employer.

Rospotrebnadzor must have information about the harmful and dangerous production factors that employees work with in the territory under its supervision. This is necessary, first of all, to obtain information on occupational diseases and their prevention.

Employees working in hazardous working conditions must periodically undergo mandatory medical examinations. For employees to undergo a medical examination, the employer must draw up an order.   • citizens who get a job with harmful and hazardous working conditions - preliminary and periodic examinations are carried out; until the citizen reaches the age of twenty-one, the inspection is carried out every year;
  • persons applying for a job that is related to the movement of transport - undergo preliminary and periodic examinations;
  • citizens working in the food industry, children's, medical and other institutions - preliminary and periodic examinations;
  • persons working in conditions of increased danger - undergo psychiatric examinations every five years.

Deadline for approval of the name list

The employer, or his authorized representative, must draw up and approve a list of names for a medical examination by order 302N at least two months before the start of the medical examination, agreed with the honey. institution.

Inspection schedule

Order 302 for medical examinations: for whom they are required

According to Art. 76 of the Labor Code of the Russian Federation, an employee who has not passed a mandatory medical examination is suspended from work. Medical examination by order 302-n can be preliminary (i.e., when applying for a job), periodic or extraordinary. Any type of inspection is organized at the expense of the employer. Surveys are carried out on a reimbursable basis by medical institutions with which companies enter into appropriate agreements. We should remember that medical examinations of workers (order 302-n speaks directly about this) are carried out by medical organizations of any form of ownership that have a license to conduct them, as well as examinations of professional suitability. Mandatory details of such contracts are:

  • subject;
  • information on the availability of a license to conduct medical activities;
  • list of paid medical services;
  • conditions, cost and terms of their implementation;
  • calculation procedure.

Stages of personnel work to implement the requirements of order 302-n

Personnel officers are guided by Appendix No. 3, which determines the conduct of a medical examination by order of 302-n. Having concluded an agreement with a medical institution, the personnel service is obliged to establish an internal procedure for referring employees to medical examinations:

  • issue an appropriate order (sample) and familiarize employees with it against signature;
  • draw up a list of employees by order 302-n (sample below) who must be sent for a medical examination. It is issued in 2 copies - for a medical institution and a department of Rospotrebnadzor. The list determines the full name of employees for whom a medical examination is mandatory, lists professions and positions, indicates a harmful production factor and the workshop or unit where the employee works. The document is approved by the head and submitted to the RPN within 10 days, and to the medical institution 2 months before the agreed date of the medical examination.

Periodic medical examination

Order 302 involves passing medical examinations, passing tests. To issue a certificate by order 302, the conclusions of a therapist, surgeon, ENT specialist, ophthalmologist, narcologist and psychiatrist are usually required, as well as blood tests and fluorography. This is a general set, which is adjusted by the addition or exclusion of specialized specialists, detailed blood tests, etc., depending on the characteristics of the industry or profession. It is important to know that a certificate issued for the first time can and should subsequently be extended in accordance with the established frequency.

This is form No. 004-P / U, relating to medical documentation, which is issued to each person who has passed a medical examination, and consists of a general and effective parts.
